CCNC PMH Care Pathways

Pregnancy Medical Home Care Pathways are developed collaboratively by the OB physician leaders throughout Community Care of North Carolina’s Networks. The intent of the PMH Care Pathways is to promote evidence-based, best practice care in North Carolina. Several PMH Care Pathways with a package of supporting materials were developed as part of the work under the North Carolina Preterm Birth Prevention TeleHealth Network. These PMH Care Pathway Packages were shared with the provider community by the OB nurse and physician leaders in each of the CCNC networks as well as on the website. Materials for providers focus on prevention of preterm birth and other factors related to adverse birth outcomes. Please refer to the CCNC PMH Care Pathways webpage to view these materials.
